Assessment & Care Plan

Our manager will arrange a convenient time to visit you at home. The Manager will discuss your personal needs and preferences with you, before suggesting a care plan tailored specifically to your requirements. This home care assessment is free of charge and at no obligation to you. To ensure that we continue to meet your needs, ongoing assessments are conducted at regular intervals. These assessments take into account any changes in your personal situation or health and we will, where necessary, adapt your care plan to reflect these changes.
